Perioperative practice: time to throttle back.

چکیده
The United States spends more on health care than other nations, yet our health outcomes remain inferior to those of many countries. Change is therefore necessary. One approach to health care reform is to identify and eliminate practices associated with high cost and limited benefit. Recent research has shown that many perioperative practices meet this definition. An opportunity thus exists for rational reduction of perioperative expenditure. Perioperative tests and treatments improve outcomes only when targeted at specific patient subsets. For example, routine perioperative stress testing provides no incremental diagnostic yield in patients at low risk for cardiac events, and indiscriminate perioperative therapy with beta-blockers can increase mortality in otherwise stable patients. Thus, many "accepted" perioperative practices conflict with the evidence and can be safely discontinued while preserving outcomes and reducing costs. Implementation of the American College of Cardiology/American Heart Association perioperative guidelines ensures cost-effective management and promises the greatest benefit for our patients. Our society demands better care at lower cost; in perioperative medicine, it is time for us to throttle back.

Robust ℋ2 static output feedback to control an automotive throttle valve
The paper presents a control strategy for an automotive electronic throttle body, a device largely used into vehicles to increase the efficiency of the combustion engines. The synthesis of the proposed controller is based on a linear matrix inequality (LMI) formulation, which allows us to deal with uncertainties on the measurements of the position of the throttle valve. The LMI approach generat...
متن کاملPreoperative Patient Assessment Practices: A Survey of Kenyan Perioperative Nurses
Background: Patients undergoing surgery are at risk of developing complications related to surgery and anesthesia amidst heightened anxiety. The key roles played by perioperative nurses in surgery, mandate them to conduct preoperative patient assessment to identify and mitigate potential complications and to plan and deliver quality perioperative care. However, paucity of literature evidencing ...
متن کاملThrottle and Preempt: A New Flow Control for Real-Time Communications in Wormhole Networks
In this papel; we study wormhole routed networks and their suitability for real-time trafic in a priority-driven paradigm. A traditional blocking $ow control in wormhole routing may lead to a priority inversion in the sense that high priority packets are blocked by low priority packets for unlimited time. This uncontrolled priority inversion causes the frequent deadline missing. This paper ther...
متن کاملAsymmetric Modelling and Control of an Electronic Throttle
This paper presents an improved model for an automotive electronic throttle inspired on the behavior observed in real-time experiments. Due to a number of issues, particularly the return-spring, the performance of the throttle valve depends on whether it is opening or closing. This asymmetric behavior was taken into account to design a mathematical model of the throttle body and to derive a non...
